For children born on or after March 17, 2008
Parents may now complete the Statement of Live Birth on-line and electronically submit it directly to Service Ontario's Office of the Registrar General. Births occurring in the City of Woodstock on or after March 17, 2008 will no longer be registered with the municipality. Instead, parents are encouraged to use the Newborn Registration Service at www.serviceontario.ca/newborn/. This new service allows parents to electronically submit the Statement of Live Birth and submit applications for a birth certificate and social insurance number at the same time.
Travel Letters are no longer available for births occurring on or after March 17, 2008. If parents are required to travel with their newborn and have not applied for a birth certificate at the time of registration, please contact the Office of the Registrar General at 1-800-461-2156 or visit www.serviceontario.ca. Canada Customs or a travel agency can also provide information regarding what documents would be required to travel with a newborn.
For children born prior to March 17th, 2008
Parents will still be required to submit in accordance with the provisions of the Vital Statistics Act, the birth of every child born in the Province of Ontario with the municipality in which the child was born within 30 days of the date of birth.
In order for the registration to be completed by the municipality, the Statement of Live Birth must be accompanied by a fee of $20.00. This payment can be made by cheque or money order payable to the City of Woodstock, if the Statement is sent via mail. If the Statement is delivered in person to the City Clerk's Department, payment by cheque, money order, cash or debit card is acceptable. If the Statement of Live Birth is not accompanied by the $20.00 fee, the registration may be delayed.
Parents are still encouraged to utilize the online registration service at www.serviceontario.ca to complete the Statement of Live Birth and complete a Birth Certificate or Social Insurance Number application. Parents will be required to print off the Statement of Live Birth and submit with payment to the City of Woodstock, Clerks Department.

Provincial Certificates
Applicants are encouraged to utilize the Service Ontario's website at www.serviceontario.ca to order certificates.
Applications to obtain an Ontario Birth Certificate, Marriage Certificate, Death Certificate, can be obtained from the City Clerk's Department or by downloading the Application here. Applicable fees charged by the Province are set out on the application form. The completed application forms, together with the appropriate fee, must be mailed to the Office of the Registrar General, 189 Red River Road, P.O. Box 4600, Thunder Bay, ON P7B 6L8. The telephone number for the Office of the Registrar General is 1-800-461-2156. The waiting period for processing an application through the mail by the Province is a minimum of 6 to 8 weeks.
